How to refer to our clinics
Please use our referral form for health care providers and fax it to one of our offices. Please fax to the location that the patient prefers (NE or NW). We will contact the patient and inform you of their appointment time and date. Both of our locations are now open.
For Patients Self-Referring:
**We Prioritize Patients with Referrals**
Self referrals are accepted but currently have long wait times (2-3 weeks)
As feeding issues can be time sensitive, we accept self-referrals.
Please note that we prioritize booking patients with referrals therefore if you do not have a referral the wait time for an appointment may be longer. We encourage you to ask your health care provider for a referral so that we may book you in as soon as possible, however if you are unable to get a referral we will be happy to help you.
